WHO: EMSISD Student athletes going into grades 7th-12th (for 26-27 school year) who plan to participate in athletics at Saginaw HS, Highland MS, or Saginaw zoned Prairie Vista MS. WALK-INS WILL NOT BE ACCEPTED.
Doctors, Physician Assistants, Physical Therapist, and Athletic Trainers from the following locations will be conducting the physicals:
- Fort Worth Orthopedics
- ATI Physical Therapy
- SporTherapy Physical Therapy
- EMSISD Athletic Trainers
Prior to May 20th, download and print your medical history and physical paperwork below. In order to not slow the flow of the event, your medical history should be filled out prior to your appointment. If you are unable to do this, the check-in table will provide additional copies to athletes.
Participants MUST PAY AHEAD of time and select a time slot. THERE WILL BE NO WALK-INS, you will be turned away at the door. Any participants that miss or are late to their selected time slot may be unable to receive a physical that day and be turned away at the door.
